Before you participate in the study, we need you to complete a consent form. We also have a photo release form, for those who are willing to let us use pictures and videos for research and educational purposes. We will have the forms on site, but if you would like to read about the study beforehand, or print out the forms and sign them before your appointment, you can access them below.
